Submit a Claim - Claim Instructions

To be eligible to receive a payment from the Settlement, you must submit a fully completed and properly addressed Proof of Claim form. The Proof of Claim form must be submitted online no later than November 7, 2020 or, if mailed, postmarked or delivered no later than November 7, 2020.

You may submit the Proof of Claim form:
